Otherwise a pseudonym is sufficient. An email address is needed for coordination for say, workshops, translations, themes, car-sharing... - Pseudonym: - Surname and christian name: - Email: - Do you wish your email address to remain of the list of PGA contacts at the end of the conference? - What are your reasons for coming to the conference? - What do you think of the PGA hallmarks? - Are you involved in local groups? a collective? If so, can you tell us something about this group? - Do you know local groups who could help the conference financially (that is, help with travel costs)? - Do you need financial help in order to attend? - Can you contribute financially with the costs of the conference? If so, how much could you give? # PARTICIPATION - Do you think you will attend for the entirety of the conference, from 19th of August to 3rd of September? - If not, for how long could you attend? - Which decentralised location do you wish to attend? This choice could be influenced either by the conditions at the location or medical needs as well as by your choice of themes, discussions, activities and actions. # VISAS - Do you need a visa to attend? - Can you pay for your visa and travel costs? - Are you insured medically? Because legally, in France, a person must be medically insured to get a visa. In the case of visa applications, it is the members of the visa team who will be responsible in cases of serious need and only during the actual event (19th August to 3rd September). - If you have insurance, does it cover accidents while you are here? If not, it's better that you get some before coming. - Do you need money to help with your medical insurance? How much? # CAR SHARING We want to adopt this as well as hitching! - What mode of transport will you use to come? - Do you wish to car share? - How many places available in your car? - Where will you depart from? - When do you leave? - When do you return? - By what route do you return? - Can you give anyone a lift from the decentralised place to the central location (Dijon) between the 28th and 30 August? - If yes, how many people can you take? # INFORMATION FOR DECENTRALISED LOCATIONS - Are you vegan, vegetarian or other? - Are you coming with children? If so, we will have childrens spaces at each location but some are more geared up for kids than others. - Please state if you cannot sleep in a tent. - Please state if you need a mattress. - Please state if you must have a room. Your reply to each of these questions will have an influence on which decentralised location you attend. - If you have dogs/pets could you come without them in the knowledge that none of the locations wish to accept pets? # MEDICAL - Do you need access facilities? - Do you have any particular health problems that we should know about? - Do you have serious allergies? If yes, to what? - Do you have any medical training/knowledge that could be useful? # TRANSLATION - What language do you speak? - Do you need translation? - Can you participate in translation during the meetings? - If yes, in what languages? # CONTENT AND AIMS - Do you wish to contribute new skills and ideas to: workshops, themes, presentations, film/dvd/projections, theatre, discussions or a particular focus which so far hasn't been mentioned in the programme? - Do you have particular aims linked to the themes we have already announced in the different locations? - Do you have any particular material needs in this regard?
