Application form for the visas

you're welcomed to send this info around to friends and collectives interested by the next pga conference

We need to organize visa work as quick as possible for the next pga conference that will take place from August 19th to September 3rd, in France.

As it is summer holidays period, it's going to take more time to get the visas, so we need to know who needs one as quick as possible and hopefully before the 1st of july.

Even if we don't have much money, we'll try to do our best to finance visas and as much as possible, travells for people from eastern/central europe who can't pay for it. So this should hopefully not prevent people from coming to the conference. It's going to be limited though.

We need :

• Family name
• Given name
• Address
• Date of birth
• Passport number
  • Date of issue
  • Date of expiry
• Address for the invitation letter

It can be usefull to have the fax et phone number from the french embassy/consulate, and a fax number where to receive the invitation letter too.
